This Privacy Notice (“Privacy Notice”) describes how SAGA Diagnostics US, Inc., and (collectively “SAGA,” “us,” “we,” “our”) collect, use, and disclose your personal information when you visit our website at https://www.sagadiagnostics.com, or any other websites, applications, social media accounts, and other online features (collectively, our “Digital Properties”) that link to this Privacy Notice. This Privacy Notice applies to our services in the United States. For information regarding our privacy practices in the European Union and European Economic Area, please refer to our European Data Privacy Statement.
This Privacy Notice does not apply to your Protected Health Information (“PHI”) that is governed by the Health Insurance Portability and Accountability Act (“HIPAA”), regardless of where it is collected. This Privacy Notice also does not apply to job applicants or our employees, contractors, or agents.
Sources of Personal Information Collected
We collect Personal Information about you from the following sources:
- Directly from you. We may collect personal information you provide directly to us through our Digital Properties.
- Automatically and through tracking technologies. We may automatically collect information about you, such as through cookies and other tracking technologies, when you interact with our Digital Properties.
- Third-party sources. We may collect personal information from third parties, such as service and content providers, publicly available sources, or social media sites.
Types of Personal Information We Collect
We may collect the following types of Personal Information:
- We collect your name, contact details such as your name, email address, physical address, and phone number.
- Device Identifiers. When you visit our Digital Properties, we collect your Internet Protocol (“IP”) device type, cookie identifiers, mobile IDs, and other data.
- Provider Information. If you register as a health care provider, we collect your organization name, affiliations, medical specialty, National Provider Identification (NPI), username or other credentials.
- Non-precise geolocation data, such as your approximate location based on your IP address.
- Network activity and usage data, such as your browsing history, ordering tendencies, pages you viewed and other details about your interactions with our Digital Properties.
- Professional or Employment-Related Information. We collect informationsuch as job titles, organization, professional licenses, credentials, affiliations, and other professional information.
- Sensitive Personal Data, such as health-related data or sensitive demographic data.
If We ask you to provide any other personal data not described above, then the personal data we will ask you to provide, and the reason why will be made clear to you at the time collected.
You may decline providing personal information and may use browsing controls to prevent automatic data collection. However, if you choose not to allow necessary information for certain Digital Property features, your experience may not work as intended. We may deidentify personal data so that it cannot reasonably be reidentified by us or by another person, and we may use this deidentified data for any reason permitted by applicable law. If we deidentify information that was originally based on personal data, we maintain and use that information in deidentified form and will not attempt to reidentify the data, except as otherwise permitted under applicable law.

SAGA uses and discloses personal data that We collect to manage our relationship with you, along with other businesses purposes. These purposes may include the following:
- Providing you with our products and client services, such as maintaining our website and personalizing your experience.
- Business operations, such as operating, evaluating, and improving our business by performing accounting, auditing, record keeping, and other internal functions; undertaking product improvement, development, and research; laboratory testing and service delivery.
- Communications and providing you with support, such as providing you with information about our products and services, addressing your inquiries and feedback, requests, and other communications you send us, and providing you with client support services.
- Marketing or Advertising, such as understanding your preferences better and to appropriately personalizing our marketing.
- Complying with laws, such as to comply with legal obligations and process, and contractual obligations, as well as our policies and terms and conditions.
Our Disclosure of Your Personal Data
There are certain circumstances under which we may share your information with certain third parties, as set forth below:
- Business Transfers:As we develop our business, we might sell or buy businesses or assets. In the event of a corporate sale, merger, reorganization, bankruptcy, dissolution or similar event, your information may be transferred to another party.

Children’s Privacy
Our Digital Properties are not directed at, marketed to, nor intended for, children under 13 years of age. We do not knowingly collect any information, including Personal Information, from children under 13 years of age. If you believe that we have inadvertently collected Personal Information from a child under the age of 13, please contact us at privacy@sagadiagnostics.com, and we will take prompt steps to delete the information.

Our Digital Properties may contain links to other websites not operated or controlled by us (“External Links”). This Privacy Notice does not apply to External Links, and we are not responsible for the privacy practices or data collection of such third-party services. Data Retention
We retain personal data as long as we have a relationship with you. Once our relationship with you has come to an end, we will retain your personal information for a period of time that enables us to:
- Maintain business records for analysis and/or audit purposes;
- Perform internal research to assess and improve our business;
- Comply with record retention requirements under the law or other relevant legal or regulatory requirements or internal document retention policy;

Once there is no longer a legitimate business purpose to retain data, we dispose of it utilizing appropriate security measures such as shredding, anonymization or other appropriate steps.

If you live in Nebraska or Texas, you may have certain rights concerning the processing and use of personal information. These rights may be subject to exemptions under applicable law. Authorized Agents: Authorized agents may exercise rights on behalf of an individual by submitting a request via privacy@sagadiagnostics.com and indicating that they are submitting the request as an agent. We may require the agent to demonstrate authority to act on behalf of the individual by providing signed permission from the individual. We may also require you to verify your own identity directly with us or to directly confirm with us that the individual provided the authorized agent permission to submit the request.
